PAYMENT POLICY
Payment for each service provided is due at the time of service. You may make payments as a self-pay client, without filing an insurance claim or you may file your insurance. If you're filing your insurance, prior authorization will be required before services are rendered. If your plan is not listed, we will be unable to file a claim on your behalf. If your insurance permits, we may be able to see you as an out of network provider. We can also provide you with a form that you can submit to your insurance company to instruct them to send reimbursements directly to you.

Please be aware that with any health plans, there may be deductibles and co-payments for which you could be responsible for and will be due at the time of service.

GOOD FAITH ESTIMATE
Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required provide a "Good Faith Estimate" (GFE) informing individuals, who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage, estimating their expected charges.
